New Patient Paperwork
What to Expect:
- Check-In & Paperwork – Bring your ID, insurance card, referral (if required), and any previous imaging or records.
- Medical History Review – We’ll ask about your symptoms, past medical history, medications, and activity level.
- Physical Examination – Dr Costa will examine you to determine a proper clinical diagnosis.
- Imaging – X-rays will be taken on site; advanced imaging (MRI/CT) may be ordered if necessary.
- Discussion – Dr Costa will explain the diagnosis and answer your questions.

- Medication
- Bracing options
- Injection options
- Surgery
- Personalized Plan – You will leave the office with a clear understanding of your diagnosis and the next steps
If Surgery is suggested:
- Scheduling – You will speak to our surgical coordinator to choose an available date that works best for you.
- Pre-admission Testing (blood work, chest X-ray, and EKG) will be arranged. These tests will need to be completed within 30 days of your surgery date
- Medical Clearances – once PATs are complete, you will follow up with your pcp and your cardiologist if necessary to obtain clearance for surgery
- Pre op Visit – 2 weeks prior to surgery, Dr Costa and his PAs will sit down with you to review the procedure, discuss any questions or concerns, and sign consents for the procedure
- Post op visit – this will be scheduled for 2 weeks following surgery.
